Wellness Program

Information about 2019 Incentives:

The City of Frisco has partnered with Viverae, a Dallas based Wellness Technology Company. The Viverae technology platform will provide a secure website for employee access, fun, engaging challenges, and resources to promote good health. The City is creating a culture that promotes healthier lifestyles and well-being. When we all work towards this goal, we help control what we pay for healthcare. Our health plans are funded by our employees and the City of Frisco. Employees who are enrolled in the City's health plan can earn up to $500 in their Health Reimbursement Account (HRA) and medical premium incentives for meeting certain requirements.

Who is eligible to participate?

Employees enrolled in one of the City's medical Plans

Participation earns the following Incentives:
  • Up to $500 towards the 2019 HRA (Deposits on January 1, 2019)
  • $480 annual savings for 2019 Medical Premium for being Tobacco-free or completing a tobacco-free program
  • $240 annual savings for 2019 Medical Premiums for meeting at least 3 measures on their biometric screening

Viverae offers additional programs to help employees earn these incentives. With the City's partnership with Cigna, members can also participate in Cigna's Tobacco-free program as well as Cigna's Healthy Pregnancy Healthy Babies program. View the documents below for details.

New Employees with a benefits effective date between 11/1/17 to 10/01/2018

As a new employee with the City, you start earning incentives by completing a biometric screening. A new employee can earn up to $500 for 2019 HRA dollars and the $240 annual savings in 2019 medical premium incentives. In addition to those incentives, the City also wants to reward new hires with the chance to also earn 2018 wellness dollars as well.

New employees completing a Biometric screening earn:
  • $500 towards 2018 HRA for new employees with benefits effective dates before April 1st, 2018
  • $300 towards 2018 HRA for new employees with benefits effective dates on or after April 1st, 2018 up until October 1st, 2018.
  • Up to $500 towards the 2019 HRA (Deposits on January 1st, 2019)
  • $240 annual savings for 2019 Medical premiums for meeting at least 3 measures on their biometric screening
New employees can also earn the additional $480 annual savings for the 2019 Medical Premium incentive by:
  • Completing the Tobacco affidavit online at their Connect Viverae portal or by completing a tobacco-free program offered through Viverae or through Cigna.

Uniformed Fire and Police:

Uniformed Fire and Police complete their biometric screening as part of the new employee process, the annual physical with Freedom Total Wellness. You can review your incentive status by logging into the Viverae portal. Note: Please allow at least 3 weeks from your benefits effective date to show on your portal. If additional incentive needs to be earned, you can also participate in other activities through Viverae as well to get the most out the incentives.

How to Get Started:

  • Register at Connect.viverae.com (use Internet Explorer as your browser)
  • The identifier = employee ID number (If the number is less than 4 digits, add leading zeros. For example, if the employee ID number was 17, then it should be entered in as 0017.)
  • The registration code = frisco
  • Create a username and password and explore your Connect Portal.

If you are on the go, members can link their devices to their Connect Portal with Viverae to participate in the challenges, view points status, and keep track of where they are in the year with their incentives.
